What is Ulthera?
Ulthera — officially named Ultherapy — is a skin-lifting technology that uses micro-focused ultrasound (MFU), made by Merz Aesthetics in the United States. It is one of the few lifting technologies cleared by the US FDA. What earned Ulthera its reputation as the gold standard of non-surgical lifting is depth: it can deliver energy down to the SMAS layer — the same layer a surgeon works on in a facelift — with no incisions and almost no recovery time.
How does Ulthera work?
The device focuses ultrasound energy into tiny heat points beneath the skin at several depths, from the superficial layers down to the SMAS at roughly 4.5 millimetres. That heat triggers the body to build new collagen and elastin to replace what has degraded with age.
What separates Ulthera from generic HIFU machines is its ultrasound imaging system: the doctor sees your actual skin layers on screen while treating, so every line of energy is placed on the target layer rather than fired blind. That precision is also why results depend so heavily on the doctor — someone has to read the image and plan the treatment lines well.

What does the procedure involve?
The doctor first assesses your skin and plans the number of treatment lines for each area. The skin is then cleansed and numbing cream applied for about 30–45 minutes. During treatment the doctor works the handpiece along the planned lines; the whole session takes roughly 45–90 minutes depending on the areas treated. Afterwards you can go straight back to your day.
Does Ulthera hurt? Is there downtime?
During the shots you feel pulses of heat or a deep ache under the skin — stronger where skin is thin or close to bone. With numbing cream, most people find it tolerable. Afterwards there may be redness, slight swelling or tenderness for a few days, which is normal. There is no need to take time off work or rest.
When do you see results, and how long do they last?
Immediately after treatment you will see an initial lift of around 20–30%, caused by the heat contracting the tissue. The real result develops gradually over 2–3 months as new collagen finishes forming, and generally lasts about 12–18 months depending on age, skin condition and aftercare. It is then common to repeat the treatment to maintain the result.
Ulthera SPT vs Ulthera Prime — what's the difference?
Ulthera Prime is the newest model, launched by Merz in late 2024. It works on the same principle as the existing SPT model but processes faster, and its larger display gives the doctor a clearer view of the skin layers; many people also find it more comfortable on the skin. The lifting results of the two models are similar — the factor that deserves the most weight is still the doctor's experience, not the machine model.
Side effects and limitations to know
Possible side effects are temporary redness, swelling or tenderness. Some people experience numbness or tingling along nerve lines, which usually resolves on its own within a few weeks. The key limitation: Ulthera is not a substitute for a surgical facelift — significantly loose skin may not respond as much as hoped. An honest assessment from the doctor before treatment matters a great deal.
